Duties

• Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform care plans.
• Administer medications, injections, IV infusions, and other treatments following established protocols and safety standards.
• Assist with diagnostic evaluations such as specimen collection, lab processing, and medical imaging procedures.
• Manage complex care needs including wound care, catheterization, tube feeding, ventilator management, and pain control strategies.
• Document all patient interactions accurately using Electronic Health Records (EHR) systems like Epic or Cerner while maintaining strict compliance with HIPAA and confidentiality policies.
• Collaborate with interdisciplinary teams to develop discharge plans, coordinate case management efforts, and ensure seamless patient transitions across care settings.
• Provide specialized care for patients with conditions such as dementia, Alzheimer's disease, developmental disabilities, infectious diseases, or those requiring hospice or palliative care.

Requirements

• Valid state licensure as a Licensed Practical Nurse (LPN) with current certification to practice.
• Proven experience in diverse clinical environments such as hospitals (including ICU or trauma centers), nursing homes, skilled nursing facilities, or home care settings.
• Demonstrated proficiency with EMR/EHR systems like Epic, Cerner, Athenahealth or eClinicalWorks for accurate documentation.
• Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/10), and medication administration protocols.
• Experience working with geriatric patients and individuals with disabilities or complex health needs such as dementia or memory care.
• Ability to perform clinical procedures including phlebotomy, suturing, sterile processing techniques, catheterization, and first aid.
• Excellent communication skills for caregiver interaction and patient education while adhering to HIPAA regulations.
• Critical thinking skills with experience in triage assessments and acute pain management in fast-paced environments such as emergency rooms or urgent care clinics.
