Licensed Practical Nurse

Job Overview
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The LPN will provide essential nursing care to patients in various settings, including patient's home and ALF's. This role requires a strong understanding of patient care standards, acute pain management, and infectious disease care.

Duties

• Provide direct patient care under the supervision of registered nurses or physicians.
• Conduct assessments and monitor patients’ health status.
• Administer medications and treatments as prescribed.
• Assist with discharge planning to ensure smooth transitions for patients.
• Educate patients and their families on health management and disease prevention.
• Collaborate with healthcare team members to develop individualized care plans.
• Ensure compliance with infection control standards and safety protocols.

Qualifications

• Current LPN license in the state of practice.
• Experience in home community care or acute care settings preferred.
• Knowledge of acute pain management strategies and discharge planning processes.
• Familiarity with infectious disease care protocols.
• Strong communication skills and ability to work collaboratively within a healthcare team.
• Proficient in using electronic health record systems, particularly Kinnser.
