Experience in SNF environments is a plus, and the role requires flexibility to work every other weekend. Duties

• Deliver direct patient care under the supervision of registered nurses and physicians, including administering medications and treatments as prescribed.
• Monitor patient health status by observing, recording, and reporting changes in condition.
• Assist with daily living activities such as bathing, dressing, and mobility support.
• Ensure all nursing procedures comply with facility policies and safety standards.
• Document patient information accurately and maintain detailed records of care provided.
• Collaborate with healthcare team members to develop and implement individualized care plans.
• Support patients emotionally and provide education on health management strategies.
• Work every other weekend to ensure continuous patient care coverage.

Skills

• Valid licensure as a Licensed Practical Nurse (LPN).
• Prior experience in a skilled nursing facility (SNF) is preferred.
• Strong clinical skills with the ability to assess patient needs effectively.
• Excellent communication and interpersonal skills to interact with patients, families, and team members.
• Ability to work independently while maintaining a team-oriented approach.
• Flexibility to work on weekends every other week.
