Job Overview

Duties

• Perform comprehensive patient assessments, including vital signs, physical examinations, and detailed medical histories to inform care plans.
• Administer medications and injections accurately following prescribed protocols, including IV infusions and medication management.
• Assist with complex procedures such as catheterization, tube feeding, sterile processing, suturing, and wound care using aseptic techniques.
• Monitor patients’ conditions closely, managing acute pain and airway issues while recognizing signs of deterioration for timely intervention.
• Document all patient interactions meticulously within electronic health records (EHR/EHR systems), ensuring compliance with HIPAA and confidentiality policies.
• Coordinate discharge planning, case management activities, and collaborate with physicians on diagnostic evaluations and treatment adjustments.
• Provide specialized care for diverse populations including geriatric patients with dementia or Alzheimer’s disease, individuals with developmental disabilities, pediatric patients in neonatal or pediatric intensive care units (NICU/PICU), and those requiring hospice or memory care services.

Qualifications

• Valid state licensure as a Licensed Practical Nurse (LPN) with current certification to practice.
• Demonstrated experience working in various healthcare settings such as long-term care facilities, hospitals (including ICU or trauma centers), emergency rooms, or home health environments.
• Proficiency in EMR/EHR systems like Epic or Cerner for accurate documentation and patient record management.
• Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/ICD-10), CPT coding, and infection control protocols.
• Experience administering injections, phlebotomy/venipuncture, catheter care, and managing ventilators or other medical equipment.
• Critical care experience such as ICU or Level I/II trauma center exposure is highly desirable.
• Ability to perform triage assessments in urgent or emergency situations with confidence and professionalism.
• Excellent communication skills for effective caregiver communication and patient education across diverse populations including seniors and individuals with disabilities.
• Knowledge of long-term care regulations and skills in discharge planning and case management are preferred.

Pay: $21.00 - $35.00 per hour
