The LPN works under the supervision of a Registered Nurse, follows the physician’s prescribed plan of care, and partners with families to support each child’s health, safety, comfort, and development.

The ideal candidate has strong clinical skills, communicates effectively, and is committed to providing attentive, individualized care.

Qualifications

  • Graduate of an approved practical nursing program.
  • Active, valid Maryland Licensed Practical Nurse license.
  • Current CPR certification.
  • Completion of required health and tuberculosis screenings.
  • At least one year of clinical experience within the past three years providing skilled nursing care to pediatric clients, preferably in home health, a hospital, or a long-term care setting.

Direct Patient Care

  • Provide individualized skilled nursing care in accordance with the physician’s plan of care and direction from the supervising RN.
  • Perform medication administration, gastrostomy and enteral feeding, suctioning, tracheostomy care, and other prescribed treatments within the LPN’s scope of practice and verified competencies.
  • Monitor vital signs, symptoms, treatment responses, and medical equipment, including pulse oximeters and oxygen concentrators.
  • Support age-appropriate activities of daily living, mobility, positioning, comfort, and developmental activities.
  • Prepare supplies and equipment using appropriate aseptic techniques and infection prevention practices.

Clinical Monitoring and Documentation

  • Observe the child’s physical and emotional condition and promptly report changes or concerns to the supervising RN.
  • Complete accurate clinical and progress notes on the day care is provided and submit documentation within agency deadlines.
  • Document nursing interventions, the child’s response to care, caregiver education, and communication with the care team.
  • Recognize urgent changes in condition and follow established emergency response procedures.

Family Support and Care Coordination

  • Reinforce caregiver education on medication administration, feeding, respiratory care, and other care techniques under RN guidance.
  • Communicate respectfully and effectively with families, caregivers, and healthcare professionals.
  • Collaborate with the RN Supervisor to maintain continuity of care and communicate scheduling or care concerns promptly.
  • Support a safe, organized, and respectful home care environment.

Professional Standards

  • Maintain pediatric nursing competencies and complete required training.
  • Participate in agency in-service education, staff meetings, and quality improvement activities.
  • Follow agency policies for patient safety, infection prevention, confidentiality, and emergency preparedness.
  • Perform nursing procedures only within the LPN’s scope of practice and demonstrated competencies.

Additional Requirements

  • Valid driver’s license and reliable transportation to assigned client homes.
  • Ability to perform essential care duties, including bending, repositioning, transferring, and supporting pediatric clients, with or without reasonable accommodation.
  • Ability to safely lift or assist with lifting 50 pounds or more, using appropriate equipment and assistance when required.
  • Dependable attendance and punctual arrival for scheduled assignments.
