What they need
YourADH is a mission-driven healthcare organization dedicated to enhancing the health, independence, and quality of life of older adults and individuals with disabilities. Through our Adult Day Health program, we provide a supportive, engaging, and person-centered environment where participants receive coordinated nursing care, therapeutic services, social engagement, nutritious meals, and meaningful activities designed to promote overall well-being.
Our interdisciplinary team works closely with participants, caregivers, healthcare providers, and community partners to ensure each individual receives compassionate, personalized care while remaining active and connected within their community.
We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our Adult Day Health team. The LPN will provide direct nursing care and clinical support to participants, helping promote their health, safety, independence, and overall quality of life.
Working closely with the RN and interdisciplinary Care Team, the LPN will implement individualized care plans, administer medications and treatments, monitor changes in participants' health and functioning, and provide appropriate clinical support. This position plays an important role in helping participants remain healthy and engaged in the community while reducing unnecessary hospitalizations and re-hospitalizations.
Key Responsibilities
- Provide direct nursing care in accordance with individualized care plans, physician orders, organizational policies, and the Massachusetts LPN scope of practice.
- Monitor participants for changes in physical, cognitive, behavioral, and functional status and communicate significant findings appropriately.
- Administer medications and treatments as ordered and monitor participants for effectiveness, adverse reactions, and other concerns.
- Collaborate with the RN and interdisciplinary Care Team to coordinate and implement participant care.
- Communicate with families, caregivers, physicians, nursing facilities, residential providers, and other community-based providers as directed.
- Maintain accurate, timely, and thorough clinical documentation in accordance with organizational policies and regulatory requirements.
- Provide health education and clinical training to participants, caregivers, and staff within the LPN scope of practice.
- Provide clinical guidance and support to program staff related to participant care.
- Monitor prescribed dietary, nutritional, and feeding requirements outlined in participant care plans.
- Follow all applicable nursing, infection prevention, safety, emergency, regulatory, and organizational policies and procedures.
- Respond appropriately to changes in participant condition and initiate interventions and notifications within the LPN scope of practice.
- Perform other duties as assigned.
Qualifications
Required:
- Current, unrestricted Massachusetts Licensed Practical Nurse (LPN) license.
- Knowledge of applicable nursing standards, regulations, and clinical practices.
- Strong communication, organization, time-management, and problem-solving skills.
- Ability to prioritize responsibilities and work effectively both independently and as part of an interdisciplinary team.
- Fluency in English.
Preferred:
- 1–2 years of nursing experience working with older adults or individuals with chronic health conditions, memory loss, or dementia.
- Previous Adult Day Health experience.
- Fluency in Cape Verdean Creole or another additional language.
Physical & Work Environment
The LPN works in a clean, well-lit Adult Day Health environment with fluctuating temperatures and close interaction with participants and staff.
The position requires:
- Frequent sitting and substantial computer use, including keyboard and mouse operation.
- Regular walking and standing throughout the workday.
- Ability to negotiate stairs.
- Ability to lift, carry, push, and pull equipment and supplies weighing up to 35 pounds.
- Adequate visual acuity and manual dexterity to safely operate clinical equipment.
- Ability to perform repetitive tasks while maintaining attention to detail and accuracy.
Why Join Our Team?
This is an opportunity to make a meaningful difference in the lives of older adults and individuals with complex or chronic health needs. As an LPN on our Adult Day Health team, you will work collaboratively with nurses, caregivers, families, providers, and other professionals to support participants in maintaining their health, independence, and quality of life.
If you are a compassionate LPN who enjoys working as part of a collaborative care team and is committed to providing high-quality, person-centered care, we encourage you to apply.
