Job Summary

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. Responsibilities

• Perform thorough patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems.
• Administer medications and injections accurately following physician orders, including IV infusions and other specialized treatments.
• Assist with diagnostic evaluations such as specimen collection, processing, and basic medical imaging procedures.
• Manage wound care, catheterization, tube feeding, and sterile procedures while maintaining strict infection control protocols.
• Monitor patients’ conditions closely, recognizing signs of acute pain or deterioration, and escalate concerns promptly to physicians or senior nursing staff.
• Provide patient education on medication administration, health coaching, and post-discharge care plans to promote ongoing recovery.
• Collaborate with interdisciplinary teams in settings such as long-term care facilities, assisted living communities, hospitals (including ICU and trauma centers), hospice care environments, and outpatient clinics.
• Document all patient interactions thoroughly in compliance with HIPAA regulations and maintain accurate medical records using EMR/EHR systems like Cerner or Athenahealth.

Qualifications

• Valid state licensure as a Licensed Practical Nurse (LPN) with current certification.
• Proven experience working in diverse healthcare environments such as medical-surgical units, emergency rooms, ICU or trauma centers, long-term care facilities, or home & community care settings.
• Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/10), and basic life support (BLS).
• Experience with IV insertion, catheter care, phlebotomy, medication administration—including injections—and specimen collection.
• Familiarity with EMR/EHR systems such as Epic or eClinicalWorks is preferred.
• Ability to perform physical examinations and assessments accurately while demonstrating excellent caregiver communication skills.
• Knowledge of infection control practices and long-term care regulations; experience working with individuals with disabilities or dementia is highly valued.
• Critical thinking skills to manage acute pain cases and urgent situations effectively; prior experience in emergency medicine or level I/II trauma centers is advantageous.

Pay: $30.00 - $37.50 per hour
