- Assist with symptom management, including pain, shortness of breath, anxiety, nausea, and other end-of-life symptoms.

- Communicate effectively with the RN case manager, medical director, hospice aides, social workers, chaplains, and other members of the interdisciplinary team.
- Report patient concerns, medication issues, safety risks, falls, and other significant events according to agency policy.
- Support the hospice philosophy by promoting comfort, dignity, patient choice, and quality of life.
- Maintain compliance with applicable hospice regulations, infection prevention practices, and agency policies and procedures.
Qualifications
- Current, unrestricted LPN license in the State of Indiana.
- Current CPR certification or willingness to obtain it as required by agency policy.
- Previous nursing experience preferred; hospice, home health, long-term care, or palliative care experience is a plus.
- Strong communication, organization, and clinical observation skills.
- Ability to work independently while collaborating effectively with the interdisciplinary team.
- Compassionate, patient-centered approach to care.
- Reliable transportation and a valid driver’s license for required patient visits.
- Ability to complete accurate and timely clinical documentation.
