Job Summary

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. Duties

• Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform care plans.
• Administer medications and injections accurately, including IV infusions and catheterizations, following strict safety protocols.
• Assist with diagnostic evaluations such as specimen collection, processing, and basic medical imaging procedures.
• Provide specialized care for patients with complex needs such as dementia, Alzheimer's disease, or memory impairments within memory or hospice care settings.
• Manage acute pain and airway issues using appropriate interventions; operate ventilators and other medical equipment as needed.
• Document all patient interactions thoroughly using Electronic Health Records (EHR) systems like Epic or Cerner while maintaining strict compliance with HIPAA regulations.
• Collaborate with multidisciplinary teams on discharge planning, case management, and client communication to ensure seamless continuity of care.

Requirements

• Valid state licensure as a Licensed Practical Nurse (LPN) with current certification in Basic Life Support (BLS) and CPR.
• Proven experience working in diverse healthcare environments such as hospitals (including Level I Trauma Centers), nursing homes, or outpatient clinics.
• Demonstrated proficiency with EMR/EHR systems like Epic, Cerner, or Athenahealth; familiarity with ICD-9/10 coding is preferred.
• Critical care experience including ICU or emergency room settings is highly desirable; experience with triage and urgent care is a plus.
• Knowledge of long-term care regulations, dementia and Alzheimer’s care practices, pain management techniques, and infection control protocols.
• Strong communication skills to effectively coordinate with patients, families, caregivers, and healthcare teams.
• Ability to work independently while maintaining attention to detail in fast-paced environments requiring sterile techniques such as suturing or sterile processing.

Pay: $32.10 - $41.47 per hour
