Licensed Practical Nurse

SUMMARY OF POSITION

The Licensed Practical Nurse (LPN) will provide nursing services to individuals served by Devoted Care Support Services under the direction and supervision of the Registered Nurse (RN). The LPN will provide direct nursing care to individuals served and train staff on the health needs of the individuals they support, as assigned by the RN. The LPN will help ensure that the agency provides the necessary supports to meet each individual's needs safely and effectively.

ESSENTIAL FUNCTIONS

The Licensed Practical Nurse's duties include, but are not limited to, the following:

• Provide direct nursing care to individuals served, as assigned by and under the supervision of the RN.
• Collect health information and observations on individuals served and report them to the RN to support admission assessments and ongoing clinical evaluations.
• Train staff regarding individuals served who have specialized medical needs, as assigned by the RN.
• Monitor and support staff in carrying out health-related tasks, and report concerns or training needs to the RN.
• Provide assistance with medical appointments, as assigned.
• Communicate with families and guardians regarding health changes and concerns, as directed by the RN.
• Promptly report any change in an individual's health status, incidents, or concerns to the RN.
• Accurately document nursing care, observations, and staff training in accordance with agency policy.
• Work within the LPN scope of practice under New Jersey law at all times.
• Comply with all federal laws, the DHS-DDD Supports Program (SP) Manual, New Jersey Division of Developmental Disabilities standards and circulars, local regulations, and Devoted Care Support Services' policies and procedures.
• Cooperate with Devoted Care Support Services and Department of Human Services staff in any inspection, inquiry, or investigation.
• Successfully complete and demonstrate proficiency in all areas of required training, as mandated by the Division of Developmental Disabilities and the State of New Jersey.
• Meet annual professional development and training requirements.

ORGANIZATION MISSION AND STRATEGY

• Work with staff to ensure that the mission of Devoted Care Support Services is fulfilled through programs, strategic planning, marketing, and community outreach.
• Implement Devoted Care Support Services' policies and procedures that carry out the organization's mission.
• Support medical and clinical training and compliance efforts so that Devoted Care Support Services can successfully fulfill its mission into the future.

REQUIRED SKILLS, EDUCATION & EXPERIENCE

The Licensed Practical Nurse must meet the following criteria:

• Must be at least 20 years of age.
• Possess an active Licensed Practical Nurse license in good standing in the State of New Jersey.
• If the position requires driving, possess a driver's license valid in the State of New Jersey with no more than 5 points listed on the Motor Vehicle Abstract.
• Submit to a criminal background check and be determined not to be disqualified by the Department of Human Services prior to working with individuals served.
• Agree to have their name checked against the Central Registry of Offenders Against Individuals with Developmental Disabilities.
• Complete an application to have their name checked against the Child Abuse Record Information (CARI) registry.
• Successfully pass Federal Exclusionary checks.
• Successfully pass pre-hire, random, and for-cause drug testing.
• Maintain confidentiality and adhere to HIPAA requirements.
• Possess excellent organizational and communication skills, both oral and written.
• Read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
• Write clear reports and correspondence.
• Speak effectively in one-on-one and group settings.
• Build effective, collaborative relationships with a wide range of constituencies, individuals, and organizations of influence, including funders, partner agencies, and volunteers.
• Working knowledge of database, internet, spreadsheet, and Microsoft Office software.

Pay: $35.00 per hour

Expected hours: 5.0 per week
