Role Description This full-time, part time and PRN, on-site Licensed Practical Nurse (LPN) role is based in Bradenton, FL and surrounding areas,focusing on delivering high-quality home healthcare services to clients in their residences. The LPN will provide direct patient care such as monitoring vital signs, administering medications, assisting with treatments, and supporting activities of daily living under the supervision of a Registered Nurse or physician. Day-to-day responsibilities include implementing individualized care plans, documenting assessments and interventions in accordance with clinical and regulatory standards, and communicating changes in client condition to the broader care team. The LPN will collaborate closely with interdisciplinary team members, educate clients and families on care instructions and health management, and promote a safe, supportive environment that aligns with Boost Home Healthcare’s compassionate care model.

Qualifications

• Active, unencumbered Licensed Practical Nurse (LPN) license in the state of Florida and graduation from an accredited practical nursing program.
• Demonstrated clinical skills in medication administration, wound care, vital sign monitoring, and support with activities of daily living in a home or clinical setting.
• Strong assessment, documentation, and care planning abilities, including accurate charting, attention to detail, and adherence to regulatory and agency standards.
• Excellent interpersonal, communication, and patient education skills, with the ability to build trust and provide empathetic, person-centered care to diverse clients.
• Ability to work both independently and as part of an interdisciplinary team, manage time effectively, and adapt to changing client needs in the home environment.
• Reliable transportation to visit clients in the Bradenton, FL area, and ability to meet any agency-specific scheduling and on-call requirements.
• Current CPR/Basic Life Support (BLS) certification and knowledge of infection control, safety protocols, and quality improvement practices.
• Previous experience in home health, hospice, or community-based care is preferred, as well as familiarity with electronic health records and basic computer skills
