Licensed Practical Nurse

Job Overview

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. I am looking for a LPN to conduct in-home supervisor visits some medication management in Brunswick, Ga and surrounding areas such as Woodbine and Kingsland, Ga. Visits are limited to one hour and it is $40 per visit.

Duties

• Perform comprehensive patient assessments, including vital signs, physical examinations, and medical history documentation using electronic health record (EHR) systems.
• Collaborate with interdisciplinary teams on discharge planning, case management, and patient education to promote optimal recovery and long-term health outcomes.
• Conduct in home patient supervisory visits and done medication management visits. Visits are 1 hour.

Requirements

• Valid licensure as a Licensed Practical Nurse (LPN) in the applicable state or jurisdiction.
• Proven experience working with geriatric patients in skilled nursing facilities or long-term care environments preferred.
• Knowledge of medical terminology, anatomy, physiology, and pharmacology related to patient assessment and medication administration.
• Ability to perform first aid, CPR (Basic Life Support), venipuncture (phlebotomy), sterile techniques, and aseptic procedures confidently.
• Strong communication skills to effectively collaborate with caregivers, patients with disabilities or developmental delays, families, and healthcare teams.
