Job Summary:

We are seeking a dedicated and compassionate Licensed Practical Nurse to join our team. As a Nurse at Fieldstone Nursing and Rehabilitation Center, you will provide high-quality patient care, working closely with our interdisciplinary team to ensure the best possible outcomes. Responsibilities:

• Provide direct patient care, including administering medications, performing treatments, and documenting patient information
• Collaborate with other healthcare professionals to develop and implement individualized patient care plans
• Monitor and report patient condition, including any changes or concerns
• Participate in shift reports, patient care conferences, and other meetings as needed
• Maintain a clean and organized workspace, adhering to infection control and safety standards
• Other duties as assigned

Required Qualifications:

• LPN license in state of practice
• Basic Life Support (BLS) certification
• Basic understanding of medical terminology and anatomy
• Ability to lift, push, and pull equipment and supplies

Rate:

• 0-1 Year: $30 LPN
• 2-3 Years: $31 LPN
• 4-5 Years: $32 LPN
• 6-7 Years: $33 LPN
• 8-9 Years: $33.50 LPN
• 10+ Years: $34 LPN
