Job Overview

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong background in patient care across various settings, including long-term care, assisted living, and acute care environments. The LPN will play a vital role in delivering safe, efficient nursing services while supporting our mission to improve community health outcomes.

Duties

• Provide direct patient care by performing physical examinations, vital signs assessment, and medication administration with precision and compassion.
• Assist with diagnostic evaluations, specimen collection, and processing to support accurate clinical assessments.
• Document medical records accurately using Electronic Health Record (EHR) systems such as Epic or Cerner while maintaining strict compliance with HIPAA regulations.
• Collaborate with multidisciplinary teams to develop and implement individualized care plans for patients with complex needs such as dementia or Alzheimer's disease.

Qualifications

• Valid state licensure as a Licensed Practical Nurse (LPN).
• Demonstrated experience working with diverse patient populations including geriatrics, pediatrics, or individuals with disabilities.
• Proficiency in EMR/EHR systems such as Epic, Cerner, or Athenahealth; experience with medical documentation is essential.
• Strong knowledge of physiology, anatomy, and medical terminology relevant to acute and chronic care settings.
• Experience in long-term care facilities, assisted living communities, or skilled nursing facilities is highly desirable.
• Ability to perform clinical procedures such as venipuncture, suturing assistance, catheterization, and first aid effectively.
• Excellent communication skills for caregiver interaction and patient education while adhering to clinical confidentiality policies.
