Licensed Practical Nurse

Job Summary

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. Duties

• Perform comprehensive physical examinations and patient assessments to determine health status and identify needs.
• Administer medications, including injections and IV infusions, following proper protocols and safety procedures.
• Monitor vital signs and manage acute pain or symptoms with appropriate interventions.
• Assist with diagnostic evaluations such as specimen collection, processing, and basic medical imaging procedures.
• Provide specialized care for patients with complex conditions like dementia, Alzheimer's disease, or those requiring memory or hospice care.
• Support discharge planning by educating patients and families on medication management, care routines, and follow-up appointments.
• Maintain accurate medical documentation within Electronic Health Record (EHR) systems while adhering to HIPAA confidentiality policies.
• Collaborate with interdisciplinary teams including physicians, case managers, and caregivers to coordinate comprehensive patient care.

Skills

• Proven experience working with diverse patient populations including geriatrics, pediatrics, or individuals with disabilities.
• Strong knowledge of physiology, anatomy, and medical terminology relevant to nursing practice.
• Proficiency in EMR/EHR systems such as Epic or Cerner for documentation and patient record management.
• Skilled in IV insertion, catheterization, phlebotomy, tube feeding, and sterile technique.
• Experience in critical care environments like ICU or Level I Trauma Centers is highly desirable.
• Ability to perform triage assessments efficiently in urgent or emergency care settings.
• Familiarity with long-term care regulations, dementia care strategies, and hospice protocols.
• Excellent communication skills for effective caregiver communication and client education.
