Responsibilities

• Deliver comprehensive nursing care by performing chronic care management plans and monitoring, examinations, vital sign assessments, and patient evaluations to determine health status and needs.
• Administer medications, injections, IV infusions, and other treatments following established protocols and safety standards.
• Monitor patients’ conditions closely, including managing acute pain, airway management, ventilator support, and infection control measures.
• Document all patient interactions accurately using electronic health record (EHR) systems , while maintaining HIPPA

Requirements

• Valid state licensure as a Licensed Practical Nurse (LPN) with current certification in CPR/BLS (Basic Life Support).
• Proven experience working in medical-surgical units, ICU settings, emergency rooms, or long-term care facilities.
• Familiarity with EMR/EHR systems for accurate documentation.
• Knowledge of medical terminology, physiology principles, ICD coding (ICD-9/10), and medication administration protocols.
• Experience with chronic. care management care
• Strong communication skills for effective caregiver-patient interactions and team collaboration.
• Ability to perform physical examinations and procedures such as IV placement and IM sq injections confidently.
• Commitment to maintaining clinical confidentiality policies and adhering to long-term care regulations.

Pay: $30.00 - $35.00 per hour
