Licensed Practical Nurse

Job Overview

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide high-quality, one-on-one home care services. In this role, you will work closely with clients in their homes, delivering personalized nursing care tailored to their individual needs. The ideal candidate will have a strong commitment to patient well-being, excellent communication skills, and the ability to work independently in a home care setting.

Duties

• Provide direct nursing care to clients in their homes, ensuring comfort and safety during each visit
• Assess clients’ health status and monitor vital signs regularly
• Administer medications and treatments as prescribed by physicians
• Develop and implement individualized care plans in collaboration with healthcare teams and family members
• Educate clients and their families on health management, medication usage, and safety procedures
• Document all care activities accurately and maintain detailed patient records
• Coordinate with other healthcare professionals to ensure comprehensive care delivery
• Maintain compliance with all safety protocols and healthcare regulations specific to home care settings

Skills

• Valid licensure as a Licensed Practical Nurse (LPN)
• Proven experience providing home health or similar one-on-one patient care preferred
• Strong assessment skills and attention to detail
• Excellent interpersonal and communication skills for effective interaction with clients and families
• Ability to work independently with minimal supervision while maintaining professionalism
• Compassionate attitude with a focus on patient-centered care
• Organizational skills to manage documentation and scheduling efficiently
• Knowledge of safety standards and infection control practices in a home environment

Pay: $33.00 - $36.00 per hour
