Licensed Practical Nurse

We’re looking for a Licensed Practical Nurse to provide nursing care in clients’ homes under the direction of an RN. This role offers the opportunity to build relationships with clients while helping meet their day-to-day health needs.

Responsibilities

• Provide nursing visits and direct care according to each client’s care plan and RN direction.
• Perform focused nursing assessments, monitor changes in condition, and promptly communicate concerns to the RN.
• Care for clients with complex medical needs, including ventilator-dependent clients and those who use specialized medical equipment.
• Respond to changes in condition and medical emergencies according to agency procedures.
• Prepare medications per the medication management plan and carry out provider orders under RN direction as needed.
• Coordinate medical appointments, follow up on care changes, and communicate with families and other health care providers.
• Monitor care staff and reinforce established care instructions as delegated by the RN.
• Maintain accurate clinical records, protect client confidentiality, and follow safety and infection prevention procedures.

Requirements

• High school diploma and graduation from a practical nursing program.
• Current Minnesota Licensed Practical Nurse license.
• Ability to work independently, follow care instructions, and communicate clearly in writing and conversation.
• A dependable, respectful approach to working with clients, families, and the care team.
• Ability to perform the physical duties of the position, including lifting, carrying, pushing, or pulling up to 50 pounds frequently and heavier loads occasionally, with or without reasonable accommodation.

Preferred experience

• Experience in home care or assisted living.

Pay: $32.00 - $34.00 per hour
