Overview

As a Licensed Practical Nurse (LPN), you’ll deliver attentive, practical nursing care and help patients and families navigate each step of treatment. You’ll assess patient needs, administer medications and treatments within your scope, and document care accurately while following clinical confidentiality policies and Health Insurance Portability and Accountability Act (HIPAA) requirements.

Responsibilities

• Measure vital signs, gather intake information, support physical examinations, and report changes in patient condition.
• Administer medications and injections, provide catheter care, support tube feeding.
• Coordinate with nurses, providers, patients, and caregivers on care plans, triage, discharge planning, and case management.
• Maintain complete medical documentation in electronic medical record (EMR) or electronic health record (EHR) systems; follow infection control and aseptic technique.
• Provide compassionate patient care across settings such as clinics, hospitals, skilled nursing facilities, long-term care, assisted living, home care, hospice, or memory care.

Skills

• Required: Current LPN license for the applicable jurisdiction; nursing knowledge, patient assessment skills, medical terminology, and accurate documentation.
• Required: Clear client and caregiver communication, sound judgment, and commitment to patient privacy and safety.
• Preferred: Experience in long-term care.
• Additional relevant experience may include developmental disabilities, Alzheimer’s or dementia care, pain management, catheterization, specimen processing, and long-term care regulations.
• Medication Administration: Accurate prep, timing, and recording of oral, topical, and injectable medications via Electronic Medication Administration Records (eMAR).
• Wound Care and dressings: Managing surgical wounds, pressure ulcers, and other skin issues.
• Point Click Care (PCC) High proficiency with specialized SNF Electronic Health Records (EHR) systems for legal documentation.
• Continuous physical monitoring, identifying critical changes in resident status, and reporting directly to RNs or Physicians.
• Infection Control knowledge.

Pay: $35.00 - $38.00 per hour
