Position Overview

All Heart Home Care is looking for dedicated Licensed Practical Nurses who are passionate about delivering quality nursing care in a home‑based setting. This role focuses on individualized patient support and meaningful nurse‑to‑client relationships. Primary Duties

• Deliver hands‑on nursing care in a residential environment

• Carry out care plans developed for each individual client

• Measure, record, and report vital signs and patient progress

• Administer prescribed medications and treatments

• Observe and report changes in health status to the supervising nurse or care team

• Maintain detailed and accurate patient records

• Provide care that respects client dignity, comfort, and independence

Required Qualifications

• Active license as a Licensed Practical Nurse

• Current certification in Cardiopulmonary Resuscitation

• Strong interpersonal and organizational skills

• Ability to manage time effectively while working independently

• Dependable transportation for travel between client homes

• Previous experience in home care or nursing services preferred, but not required
