Position Overview

We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our assisted living team in Staten Island. The LPN will provide quality nursing care to residents, administer medications, monitor residents' health, and work closely with residents, families, caregivers, and other healthcare professionals.

This is a Monday–Friday, 11:30 AM–8:00 PM position and is ideal for an LPN looking for a consistent weekday schedule.

Key Responsibilities

• Provide direct nursing care to residents in an assisted living environment.
• Administer medications and treatments in accordance with physician orders and facility policies.
• Monitor residents' health status and identify changes in condition.
• Complete nursing assessments and accurately document resident care.
• Communicate changes in residents' conditions to physicians, families, and appropriate members of the care team.
• Assist with care planning and implementation of individualized resident care plans.
• Monitor vital signs and other clinical indicators as needed.
• Respond appropriately to resident emergencies and medical concerns.
• Coordinate with physicians, registered nurses, nursing assistants, and other healthcare professionals.
• Maintain accurate and timely medical records and documentation.
• Provide education and support to residents and families regarding care and medications.
• Follow all applicable state regulations, facility policies, and infection-control procedures.
• Promote resident safety, dignity, independence, and quality of life.

Qualifications

• Current and active New York State LPN license.
• Graduate of an accredited practical nursing program.
• Previous experience in assisted living, long-term care, skilled nursing, or a similar setting preferred.
• Strong clinical assessment and communication skills.
• Ability to work independently while also functioning effectively as part of a healthcare team.
• Strong organizational and documentation skills.
• Compassionate, professional, and dependable approach to resident care.
• Ability to handle multiple priorities in a fast-paced healthcare environment.

Schedule

Monday–Friday
11:30 AM–8:00 PM

No weekend schedule is required for this position.
