Responsibilities

• Provide direct patient care, including administering medications, performing vital signs, and monitoring resident conditions
• Assist with activities of daily living (ADLs), such as bathing, dressing, and feeding
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Communicate effectively with residents, families, and healthcare staff to ensure seamless care coordination
• Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
• Document resident care and progress in accordance with facility policies and regulations

Requirements

• Current licensure as a Licensed Practical Nurse (LPN) in the state of Nebraska
• High school diploma or equivalent required; degree in nursing or related field preferred
• Minimum 1 year of experience as an LPN; experience in a long-term care setting preferred
• Strong communication, interpersonal, and teamwork skills
• Ability to lift, transfer, and move patients with care and attention
• Familiarity with electronic medical records and basic computer skills
