Licensed Practical Nurse (LPN)

Hennis Care Centre of Bolivar is a family-owned and operated skilled nursing facility. Our facility has four nursing units: Transitional Therapy, Rehab, Long-term Care and Memory Care. We strive to provide our residents and employees with the best care possible.

We are looking for a reliable Licensed Practical Nurse (LPN) to take excellent care of our residents while working together as a team, where you will:

• Advocate for the needs of the residents and/or their families
• Supervise and oversee the work of STNAs
• Ensure that care plans are carried out appropriately
• Perform routine nursing measures, such as vital signs, weights, food intake, and behavior monitoring
• Administer medications and treatments
• Perform diagnostic tests as ordered
• Document resident information in an electronic charting system
• Coordinate resident care in conjunction with the interdisciplinary team to ensure quality care
• Effectively make decisions to maximize the best clinical outcome for the resident
• Be accountable for nursing compliance, excellence, and delivery of resident care in adherence to the organization, local, state, and federal regulations
• Complete admissions and discharges in a timely manner, including documentation
• Assure HIPAA requirements are followed at all times

Qualifications:

· Current LPN license in the state of Ohio is required.

· Prior experience in long term care/skilled nursing is a plus, but not required

· All applicants must complete a pre-employment physical as well as a drug and alcohol screen.

· All applicants must have a criminal background check prior to employment.

Full-time: 7p-7a

PRN: 7p-7a

Pay: $24.00 – 26.50 per hour
