What You'll Do

As a Home Health LPN, you will provide skilled nursing care under the direction of an RN and physician-established plan of care, including:

-

Complete skilled nursing visits in patients' homes

-

Monitor and document patients' conditions

-

Provide medication management and education

-

Perform wound care and other ordered treatments

-

Monitor vital signs and report significant changes

-

Reinforce disease-management education

-

Educate patients, families, and caregivers

-

Communicate changes in condition to the supervising RN and care team

-

Coordinate with other members of the interdisciplinary team

-

Follow established plans of care and physician orders

-

Complete accurate and timely documentation using WellSky

What We're Looking For

-

Active Virginia Licensed Practical Nurse (LPN) License Required

-

Graduate of an accredited Practical Nursing program

-

Valid Driver's License and reliable transportation

-

Strong clinical and communication skills

-

Dependable, organized, and comfortable working independently in patients' homes

-

Ability to follow established plans of care and communicate effectively with the supervising RN

-

Home Health Experience Preferred, but Not Required

-

WellSky Experience is a Plus
