Responsibilities:

• Provide high-quality nursing care to our residents, including administering medications, wound care, and chronic disease management
• Develop and implement individualized care plans in collaboration with our interdisciplinary team
• Monitor and report changes in resident conditions, and make recommendations for interventions and treatments
• Perform vital signs, take inventories of medications, and maintain accurate records
• Collaborate with our rehabilitation team to develop and implement treatment plans to promote resident independence and well-being
• Participate in ongoing education and training to stay current with industry standards and best practices

Requirements:

• Current LPN licensure in the state of Massachusetts
• High school diploma or equivalent required; associate's or bachelor's degree in nursing or related field preferred
