Licensed Practical Nurse (LPN)

Job Overview

We are seeking dedicated RNs or LPNs to provide exceptional patient care across various care settings, including long-term care, acute care, and specialized units. The ideal candidate will possess strong clinical skills, a commitment to patient safety, and the ability to adapt to various care settings. You will play a vital role in promoting health, managing symptoms, and supporting patients through their recovery or ongoing health journey.

Responsibilities

• Deliver direct patient care by assessing needs, developing care plans, and implementing interventions aligned with best practices and regulatory standards.
• Administer medications, including injections and IV infusions, ensuring accuracy and adherence to protocols.
• Monitor vital signs, perform physical examinations, and document findings accurately within electronic health record (EHR) systems.
• Assist with diagnostic evaluations such as specimen collection, processing, and interpreting results for appropriate clinical action.
• Manage complex cases involving acute pain, infectious diseases, dementia or Alzheimer's care, and trauma-related conditions.
• Collaborate with interdisciplinary teams to coordinate discharge planning, case management, and long-term care strategies for optimal patient outcomes.
• Maintain strict compliance with HIPAA regulations and clinical confidentiality policies while ensuring a safe environment for patients and staff.
• Provide education and support to patients and their families regarding medication administration, disease management, and health coaching.

Qualifications

• Valid RN or LPN licensure with current state certification; additional certifications such as BLS (Basic Life Support) are required.
• Proficiency with EMR/EHR systems like PCC; familiarity with ICD coding (ICD-9/10) is preferred.
• Strong knowledge of physiology, anatomy, pharmacology, and medical terminology relevant to diverse patient populations.
• Experience working with individuals with disabilities or developmental delays; geriatric or memory care experience is advantageous.
• Ability to perform advanced nursing procedures including catheterization, tube feeding management, sterile techniques, and ventilator support if applicable.
• Excellent communication skills coupled with a compassionate approach to patient-centered care in fast-paced environments such as urgent care or skilled nursing facilities.
