Looking for a nursing role where you can focus on each patient during their visit? Elara Caring is seeking a Licensed Practical Nurse to serve patients in Avon, Lorain, and Wellington, Ohio.

You’ll provide care in patients’ homes, build relationships with patients and families, and help them manage their health in familiar surroundings. You’ll partner with an RN Case Manager and care team to support each patient’s individual needs.

Make a Difference During Every Visit

• Provide nursing care following individualized care plans and physician orders.
• Monitor patient progress and report changes or safety concerns.
• Teach patients and caregivers self-care techniques to support their health.
• Partner with the RN Case Manager to update care plans as patient needs change.
• Maintain accurate medication profiles and document care according to agency guidelines.
• Communicate with physicians, patients, families, and the care team.
• Help prevent avoidable hospitalizations and emergency room visits through proactive care.

What You’ll Need

• Current Ohio LPN license.
• Graduation from an accredited certificate, diploma, or associate nursing program.
• At least 1 year of experience in a clinical care setting.
• Home health or hospice experience preferred.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver’s license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
