What they need

Description:

Wellesley Family care is a thriving primary care practice based in Needham, Massachusetts. We are a private (independent) practice affiliated with Mass General Brigham with a focus on individualized evidence-based care for the whole family. We also highly value teamwork and collaboration and have built a team culture of safety, competence, and fun. We currently have twenty-two primary care providers on our team and have a strong growth trajectory into new business and practice initiatives. We are looking for a compassionate, detail-oriented Licensed Practical Nurse (LPN) who enjoys hands-on patient care to join our practice.

Position Summary

The Licensed Practical Nurse (LPN) is a full-time, fully in-office member of our clinical team, on site five days per week. This role is centered on hands-on, in-person nursing care. The LPN’s primary focus is conducting scheduled nurse visits and performing point-of-care testing (POCT) for our patients. When nurse visits and testing are complete, the LPN supports the nursing team with protocol-based telephone triage, helping patients get the right level of care at the right time.

The LPN works under the direction of our providers and registered nurses and within the scope of practice defined by the Massachusetts Board of Registration in Nursing. This is an excellent opportunity for an LPN who enjoys direct patient contact, takes pride in accuracy, and wants to be part of a collaborative, supportive primary care team.

  • Perform nursing procedures such as suture and staple removal, ear lavage, TB skin test placement and reading, and EKGs.
  • Conduct Medicare Annual Wellness Visits under the direct supervision of a physician, including administering the health risk assessment, updating medical, family, and medication history, obtaining vitals and measurements, completing standardized screenings (e.g., depression, fall risk, cognitive, and functional ability), and reviewing preventive care needs, with the personalized prevention plan reviewed and finalized by the supervising physician.
  • Reinforce patient education provided by the care team.
  • Recognize and promptly escalate abnormal findings or changes in patient condition to a provider or RN.
  • Collect specimens and perform point-of-care tests, which may include rapid strep, influenza, COVID-19, RSV, urinalysis dipstick, urine pregnancy, blood glucose, hemoglobin A1c, hemoglobin.
  • Follow manufacturer instructions and practice policies for every test; run and document quality control checks as required.
  • Accurately record results in the electronic health record and flag critical or abnormal values to the provider immediately.
  • Maintain testing supplies and reagents, monitor lot numbers and expiration dates, and support CLIA compliance and competency documentation.

Nurse Phone Triage (As Time Allows)

  • During time not committed to nurse visits or testing, support the triage nursing team by a returning patient phone calls and answering patient portal messages.
  • Use practice-approved, standardized triage protocols to gather information about symptoms and direct patients to the appropriate level of care (same-day visit, routine appointment, urgent care, emergency department, or 911).
  • Escalate calls requiring clinical assessment or judgment to a RN or provider in accordance with LPN scope of practice.
  • Communicate provider instructions and test results to patients as directed.
  • Document all calls clearly and completely in the electronic health record.
