• Providing direct patient care, including administering medications, performing vital signs, and assisting with daily living activities
• Developing and implementing individualized care plans in collaboration with the interdisciplinary team
• Participating in care conferences and contributing to the development of treatment plans
• Maintaining accurate and detailed records of patient care and progress

Requirements:

• Current LPN licensure in the state of Ohio
• High school diploma or equivalent required
• Previous experience as an LPN or related healthcare experience
• Strong communication and interpersonal skills
