About the Position

Love Thy Neighbor Home Health is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to provide skilled nursing care to patients in their homes under the direction of a Registered Nurse and in accordance with the patient's established plan of care.

The ideal candidate is patient-focused, organized, comfortable working independently in the field, and committed to providing safe and compassionate care.

Responsibilities

• Provide skilled nursing visits to patients in their homes according to the established plan of care and physician/provider orders.
• Monitor and document patients' vital signs, symptoms, and general condition.
• Administer medications and treatments as ordered and permitted within the LPN scope of practice.
• Perform wound care and other nursing procedures as assigned and within scope.
• Observe patients for changes in medical condition and promptly report concerns to the supervising RN or appropriate clinical team member.
• Reinforce patient and caregiver education regarding medications, treatments, diet, safety, and disease management.
• Assist patients and families in understanding and following the established plan of care.
• Maintain accurate and timely clinical documentation for each patient visit.
• Communicate effectively with the supervising RN, physicians/providers, patients, families, caregivers, and other members of the care team.
• Follow infection-control procedures and patient safety standards.
• Participate in care coordination and case communication as required.
• Follow agency policies, HIPAA requirements, and applicable Florida nursing and home health regulations.
• Notify the supervising RN promptly of hospitalizations, falls, medication concerns, changes in condition, or other significant patient issues.

Qualifications

• Current and active Licensed Practical Nurse (LPN) license in Florida or qualifying multistate nursing license permitting practice in Florida.
• Graduate of an approved practical nursing program.
• Current CPR/BLS certification.
• Ability to successfully complete required background screening.
• Valid driver's license and reliable transportation.
• Ability to travel to patients' homes within the assigned service area.
• Strong communication and documentation skills.
• Ability to follow established plans of care and clinical instructions.
• Compassionate and professional approach to patient care.

Preferred Qualifications

• Previous nursing experience preferred.
• Home health, skilled nursing, rehabilitation, assisted living, or community-based care experience is a plus.
• Experience with electronic medical records is preferred.
• Wound care, medication management, and chronic disease management experience is a plus.

Home health experience is preferred but not required. Qualified nurses who are willing to learn and are comfortable providing care in patients' homes are encouraged to apply.

Work Setting

This is a field-based home health nursing position. The LPN will travel to patient residences within Love Thy Neighbor Home Health's approved service area and provide care under the direction of the supervising RN and established plan of care.
