We are looking for a caring and dependable Licensed Practical Nurse (LPN) to join our skilled nursing and rehabilitation team for the evening/night shift. If you are passionate about helping others and want to make a difference in the lives of seniors, we want to meet you.

Key Responsibilities

• Help develop and carry out care plans for residents.
• Supervise and assist Certified Nursing Assistants (CNAs).
• Monitor residents' health and safety, including falls and skin care programs.
• Provide direct patient care and assist with daily medical needs.
• Complete and update resident evaluations and documentation.
• Support overall care goals and perform other duties as needed.

Qualifications

• Current Licensed Practical Nurse (LPN) license in good standing.
• Experience in skilled nursing or rehabilitation preferred.
