Licensed Practical Nurse (LPN) - Part-Time 2nd Shift

Sign-On Bonus: You will be eligible to receive a total sign-on bonus of $2,400, paid in four installments of $600 each. Payments will be made upon completion of 3 months, 6 months, 9 months, and 12 months of employment, in accordance with company policy.

Starting Pay: $34 per hour (Pay is based on previous experience)

$2 PM/Night shifts differential

10% weekend pay differential for all shifts

Full-time role (1.1FTE) 12 hours Overnight 6pm - 6am shifts Rotating Weekends (Skilled Rehab)

Full-time role (0.8FTE) 8 hours Mid-Shift 11:30am -7:30pm shifts Rotating Weekends (Admission Nurse)

Full-Time role (0.8FTE) 8 hours PM Shift 2:30pm-10:30pm Rotating Weekends- (Assisted Living)

Part-time role (0.6FTE) 8hours AM Shift 6:30am- 2:30pm Rotating Weekends (Assisted Living)

Part-time role (0.6FTE) 8hours PM Shift 2:30pm- 10:30pm Rotating Weekends (Assisted Living)

What you bring to the table:

• LPN License in good standing in Illinois
• Long-term Care experience (Preferred)
• Caring and compassionate nature
• Possess strong computer skills:

Duties and Responsibilities:

• Perform ongoing assessments and observations of the residents
• Dispensing and recording of all medications accurately
• Carrying out all physicians orders accurately
• Ensures that the care of the residents from CNA's and nurse is delivered appropriately
