Licensed Practical Nurse (LPN) Weekends

Overview Catholic Health Home Care is looking for passionate, dedicated LPNs to provide comprehensive care to patients at home.

Team 5: Bohemia, Brentwood, Fire Island, Great River, Islandia, Islip , Lake Ronkonkoma, Oakdale, & West Sayville.

• EVERY Weekend Shift - 7:00AM to 5:30PM ( 10 Hr Shift)
• Eligible for full time benefits

Job Details Our Home Care LPN’s

• Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients.
• Measure vital signs, Administer IVs, change dressings, clean wounds as needed
• Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations.
• Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload.
• Provide patient and family education for disease management and health promotion as directed by physician’s plan of treatment.

Minimum Qualifications

• New York State Licensed Practical Nurse (LPN) license
• Minimum of one year of recent hands on experience
• Basic Life Support Certification

Salary Range USD $35.00 - USD $45.00 /Hr.
