About the Role

Optum NY is seeking a Licensed Practical Nurse (LPN) to join its Pulmonology team in Rock Hill, NY. In this role, you will provide direct patient care, support providers during clinical procedures, and collaborate with a multidisciplinary care team to deliver high-quality, coordinated care. Key Responsibilities

• Provide nursing care through direct patient interactions and telephone communication following established clinical standards.
• Coordinate care with physicians, providers, and other healthcare team members to support seamless patient care delivery.
• Encourage patient participation in care planning and treatment goals.
• Assist physicians and providers with clinical procedures.
• Educate patients and families on treatment plans and health-related topics.
• Provide direction and guidance to clerical staff, medical assistants, and other non-licensed personnel.

Required Qualifications

• Graduate of an accredited School of Practical Nursing.
• Current New York State (NYS) Licensed Practical Nurse license with active NYS registration in good standing.

Preferred Qualifications

• Current CPR/BLS certification.
• 1+ years of experience as a Licensed Practical Nurse, or
• 2+ years of experience as a Medical Assistant in a relevant practice area.

Compensation & Benefits

Pay Range: $20.38 - $36.44 per hour
