Licensed Practical Nurse / RN

Job Overview

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our dynamic healthcare team. In this vital role, you will provide high-quality patient care across a variety of settings, including long-term care facilities, assisted living communities, hospitals, and outpatient clinics. Your expertise will ensure that patients receive personalized treatment, comfort, and support while maintaining compliance with healthcare regulations. Responsibilities

• Conduct comprehensive patient assessments, including vital signs, physical examinations, and medical history documentation using electronic health record (EHR) systems such as Epic or Cerner.
• Administer medications and injections accurately, including IV infusions and catheter care, following strict safety protocols and HIPAA confidentiality policies.
• Manage complex care needs such as Alzheimer’s care, dementia care, memory care, hospice support, and acute pain management for diverse patient populations.
• Perform diagnostic evaluations and coordinate with interdisciplinary teams for case management, discharge planning, and post-acute care.
• Assist with specialized procedures like tube feeding, venipuncture, sterile processing, wound suturing, and airway management in accordance with clinical guidelines.
• Provide patient education on medication administration, health coaching, and self-care techniques tailored to individual needs.
• Ensure meticulous medical documentation of patient care activities while adhering to long-term care regulations and ICD coding standards (ICD-9/ICD-10).

Experience

• Proven experience working with geriatric patients in skilled nursing facilities or long-term care environments.
• Critical care experience in settings such as ICU or Level I/II trauma centers is highly desirable.
• Background in hospital medicine, emergency medicine, or urgent care with familiarity in acute care procedures like spinal taps or gastric lavage.
• Hands-on experience with EMR/EHR systems such as Epic, Cerner, Athenahealth or eClinicalWorks is required for efficient documentation.
• Knowledge of infectious disease protocols, infection control practices, and medical imaging procedures enhances your ability to deliver comprehensive patient care.
• Experience working with individuals with disabilities or developmental challenges including behavioral health support is a plus.
• Certification in Basic Life Support (BLS), Advanced Cardiovascular Life Support (ACLS), and other relevant credentials are essential for ensuring readiness in emergency situations.

Pay: $30.00 - $40.00 per hour
