About the Position

Willowick Assisted Living Moments is hiring two dependable, compassionate, clinically confident Licensed Practical Nurses (LPNs) to join our memory care team. This position provides hands-on nursing care to residents while serving as a clinical leader and supervisor for the care staff. The primary location is 304 Ogden Ave. Clinton, WI

Our LPNs play an important role in ensuring residents receive safe, consistent, person-centered care while maintaining communication with physicians, families, and the interdisciplinary team.

What You'll Do

• Administer medications safely and accurately according to physician orders and facility policies
• Provide hands-on bedside care and support to residents
• Monitor residents for changes in condition and promptly communicate concerns
• Communicate significant changes in resident conditions to the appropriate clinical team members, physician, family, or responsible party
• Process and implement physician orders
• Maintain and update Individual Service Plans (ISPs) as required
• Complete required nursing documentation and maintain accurate resident records
• Monitor resident needs and ensure appropriate follow-through by the care team
• Provide direction, supervision, leadership, and support to caregiving staff during the shift
• Delegate and oversee resident care responsibilities appropriately
• Assist care staff with questions, resident needs, and changes in condition
• Promote a safe, respectful, and supportive environment for residents
• Work collaboratively with leadership, families, physicians, pharmacies, and other healthcare professionals
• Follow all applicable CBRF, DHS, medication administration, and facility policies and procedures
• Respond appropriately to resident emergencies and changes in health status
• Serve as a clinical resource and leader for the care team

What We're Looking For

• Current Wisconsin LPN license in good standing
• Experience in assisted living, memory care, long-term care, or a similar setting preferred
• Strong assessment and clinical judgment skills
• Ability to recognize and communicate changes in resident condition
• Strong medication administration skills
• Ability to lead and supervise a team of caregivers
• Excellent communication and documentation skills
• Dependable, organized, and able to work independently
• Compassionate approach to working with older adults, particularly individuals living with dementia
• Ability to remain calm and professional in challenging situations
• Commitment to providing respectful, person-centered care
