At Henderson Health & Rehabilitation, we're looking for a compassionate, driven, and resident-focused Licensed Social Worker to join our team. In this role, you'll have the opportunity to positively impact the lives of residents and families by providing emotional support, care planning, advocacy, and guidance throughout their healthcare journey.

What You'll Do

· Support residents and families with emotional, social, and discharge planning needs.

· Coordinate care plans and collaborate with interdisciplinary team members.

· Assist residents with Medicare, Medicaid, and community resource navigation.

· Complete psychosocial assessments and maintain accurate documentation.

· Advocate for resident rights, dignity, and quality of life.

· Participate in care conferences, survey readiness, and quality improvement initiatives.

· Build strong relationships with residents, families, healthcare providers, and community partners.

What We're Looking For

· Current Nevada Licensed Social Worker (LSW) or higher-level social work license.

· Bachelor's degree in Social Work or related human services field.

· At least one year of experience in healthcare, skilled nursing, or long-term care settings preferred.

· Strong communication, organization, and problem-solving skills.

· Passion for serving seniors and helping others thrive.
