Licensed Social Worker

Job Summary

We are looking for a compassionate and dedicated Licensed Social Worker (LSW) to join our Home Health & Hospice team. This position provides an opportunity to make a meaningful difference in the lives of patients and their families during some of life's most important and challenging moments.

As a Medical Social Worker, you will work alongside nurses, therapists, aides, providers, and other members of the interdisciplinary team to help patients remain safely and comfortably in their homes while receiving the care they need.

What You'll Do

• Provide psychosocial assessments and supportive counseling to patients and families.
• Help patients and caregivers navigate the emotional, social, and practical challenges associated with illness, aging, disability, and end-of-life care.
• Assist patients and families with community resources, financial assistance, advance planning, transportation, and other needs.
• Provide grief and emotional support to patients and their loved ones.
• Participate in interdisciplinary care planning and communicate with the healthcare team regarding patient needs.
• Advocate for patients and help identify barriers to receiving appropriate care.
• Coordinate referrals to appropriate community and social service resources.
• Complete required documentation in a timely and accurate manner.
• Provide compassionate support to hospice patients and families as they navigate end-of-life decisions and experiences.
• Travel to patients' homes throughout the assigned service area.

What We're Looking For

• Current Idaho Licensed Social Worker (LSW) license in good standing.
• Bachelor's or Master's degree in Social Work from an accredited program.
• Strong communication, organization, and interpersonal skills.
• Ability to work independently while collaborating effectively with an interdisciplinary team.
• Reliable transportation and willingness to travel to patients' homes.
• Valid driver's license and appropriate auto insurance.

Pay: From $29.00 per hour
