Responsibilities

• Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using Electronic Health Record (EHR) systems such as Epic or Cerner.
• Administer medications and injections accurately following physician orders, including IV infusion and catheterization procedures.
• Assist with medical procedures such as wound care, suturing, tube feeding, and sterile processing techniques.
• Monitor patients' conditions closely, utilizing critical care experience or ICU background to identify changes in status promptly.
• Collaborate with interdisciplinary teams to develop and implement individualized care plans for patients with diverse needs such as dementia, Alzheimer's disease, or developmental disabilities.
• Conduct diagnostic evaluations like laboratory specimen collection/processing and basic medical imaging support when required.
• Maintain detailed medical documentation ensuring compliance with HIPAA regulations and ICD coding standards (ICD-9/ICD-10).
• Provide patient education on health coaching topics including medication management, nutrition, discharge planning, and chronic disease management.
• Support emergency response efforts by managing airway management, ventilator use, or acute pain management protocols in urgent or hospital settings such as Level I Trauma Centers or Emergency Rooms.

Requirements

• Valid state licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN).
• Proven experience in diverse clinical environments such as hospital medicine, medical-surgical units, ICU, emergency medicine, or long-term care facilities like nursing homes or assisted living communities.
• Demonstrated proficiency with EMR/EHR systems including Epic, Cerner, Athenahealth, or eClinicalWorks.
• Strong knowledge of physiology, anatomy, pharmacology, and medical terminology relevant to patient assessment and treatment procedures.
• Experience with specialized areas such as hospice care, geriatrics, pediatrics (including neonatal ventilator), infectious disease care, or behavioral health is highly desirable.
• Ability to perform venipuncture, phlebotomy, injection administration, catheter care, and sterile techniques confidently.
• Excellent communication skills to effectively triage patient needs and collaborate with healthcare teams across multiple disciplines.
