• 7:00 AM – 3:00 PM
• 3:00 PM – 11:00 PM
• 11:00 PM – 7:00 AM

Job Overview
Autumn Lake Healthcare is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our Long-Term Care team. The LPN will deliver quality nursing care to residents with chronic conditions, disabilities, and long-term healthcare needs while promoting comfort, dignity, and safety.

Key Responsibilities

• Perform comprehensive resident assessments and monitor health status
• Develop, implement, and evaluate individualized care plans
• Administer medications and treatments per physician orders
• Document vital signs, treatments, and changes in condition accurately
• Provide emotional support and education to residents and families
• Collaborate with interdisciplinary healthcare teams
• Maintain compliance with clinical and regulatory standards

Qualifications

• Active LPN license in Maryland or Compact State
• Long-term care or geriatric nursing experience preferred
• Strong clinical, communication, and documentation skills
• Full 100 % Credentials is a Must to get hired.
• Flexible schedule also available.
