Long Term Care Manager

Pay Range: $26 - $29/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities:

• Confirm eligibility and validate events for reporting upon receipt of new case assignments
• Review historical events to determine if trend analysis is required
• Maintain internal documentation regarding the investigation process status
• Collaborate with Service Coordinators throughout the investigation process
• Evaluate open cases for potential closure based on established criteria

Key Requirements and Technology Experience:

• 2+ years of experience and knowledge of waiver processes
• Clinical background with case management experience
• Knowledge of waiver experience
• Nice to have: Background in Nursing, Social Work, or Case Management
• Preferred: Licensure as a Nurse and/or Social Worker
