Job Description

A&A Health Services, Inc. is seeking compassionate, dependable, and dedicated Licensed Practical Nurses (LPNs) to join our Home Health team.

The ideal candidate will provide high-quality nursing care to patients in the comfort of their homes while working under the supervision of a Registered Nurse (RN) and following established care plans.

Responsibilities

• Provide skilled nursing care to patients in their homes.
• Monitor and document patients’ conditions and vital signs.
• Administer medications and treatments as prescribed and within the LPN scope of practice.
• Perform wound care and dressing changes.
• Assist with catheter care and other skilled nursing procedures as authorized.
• Educate patients and families on proper care and disease management.
• Report changes in patient conditions to the supervising RN or physician.
• Complete accurate and timely clinical documentation.
• Follow infection control procedures and agency policies.
• Collaborate with the healthcare team to ensure quality patient care.

Requirements

• Active and unrestricted Florida LPN license.
• Current CPR/BLS certification.
• Previous home health experience preferred.
• Strong communication and organizational skills.
• Ability to work independently and manage assigned patient visits.
• Reliable transportation and willingness to travel to patients’ homes.
• Knowledge of home health documentation and electronic medical records preferred.
