Job Summary
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality patient care in a Family Practice setting, ensuring the well-being of patients.

Responsibilities

• Face-to-face & telephonic communication
• Perform phone triage, patient education and coaching
• Use of disease management interventions to engage patients in their health care
• e-prescribe
• Immunizations
• EMR experience
• Vital signs
• Medication reconciliation
• Specimen collection with in-house testing
• EKGs and spirometry
• Participate in the development and implementation of individualized care plans.

Skills

• Excellent communication skills for effective interaction with patients, families, and team members.
• Ability to work collaboratively within a team environment.
• Strong organizational skills with attention to detail in clinical documentation.
• Compassionate approach to patient care with a focus on empathy and support.
