Job Summary:

Homestead Health Care Center is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our dynamic team of healthcare professionals. As an LPN at Homestead, you will be responsible for providing exceptional care to our residents, conducting assessments, and managing their daily needs. We are looking for a caring and patient individual with a strong work ethic, integrity, and competence to join our team.

We are hiring part-time on the following shifts:

• Days (6:30am - 6:45pm)
• Nights (6:30pm - 6:45am)

Responsibilities and Duties:

• Conduct assessments, follow protocol, report medical or behavioral changes, and document findings
• Perform admission duties, including completing assessments, MAR, TAR, POS, reviewing chart set-up, and ordering medications
• Manage discharges, including completing documentation, notifying physicians and guardians, and arranging transportation
• Provide wound care, following protocols, irrigating, packing, and removing dressings
• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies
• Respond effectively to emergency situations

Requirements:

• LPN license
• Ability to work in a fast-paced environment with a high level of patient care
• Strong communication and interpersonal skills
• Ability to work as part of a team and lead others
• Strong attention to detail and organizational skills
