Position Summary
We’re looking for a compassionate, dependable LPN to provide skilled nursing visits and skilled respite care under the direction of a Registered Nurse. You’ll work independently in the field, build real relationships with patients and families, and be a key part of an interdisciplinary team that includes RNs, therapists, and home health aides.

Responsibilities

• Provide skilled nursing care per the physician-ordered plan of care and RN supervision
• Perform wound care, medication administration and management, vital signs, and patient assessments within LPN scope
• Provide skilled respite care shifts for qualifying patients
• Educate patients and caregivers on disease management, medications, and safety
• Report changes in patient condition promptly to the supervising RN and physician
• Document visits accurately and on time in our EMR (Alora Plus) and clock visits through EVV
• Participate in care coordination and case conferences
• Follow infection control, safety, and HIPAA policies

Qualifications

• Current, active Florida LPN license (or multistate compact license)
• Current CPR/BLS certification
• Minimum 2 year of nursing experience; home health experience strongly preferred
• IV certification a plus
• Valid driver’s license, reliable transportation, and current auto insurance
• Comfortable using a smartphone/tablet for documentation
• Strong communication skills, sound judgment, and the ability to work independently
