LPN (Licensed Practical Nurse)

Job Summary
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality nursing care to patients in various settings, including hospitals, clinics, and long-term care facilities. As an LPN, you will work under the supervision of registered nurses and physicians, assisting in patient assessments, administering medications, and ensuring the comfort and well-being of patients.

Duties

• Provide direct patient care including monitoring vital signs and assisting with daily living activities.
• Administer medications and treatments as prescribed by healthcare providers.
• Conduct physical examinations and document patient health histories.
• Assist in the management of patients with specific conditions such as dementia and those requiring infant care.
• Support airway management procedures as necessary.
• Participate in sterile processing of medical instruments to ensure a safe environment for patient care.
• Collaborate with healthcare team members to develop and implement patient care plans.
• Educate patients and their families about health conditions, treatment plans, and preventive measures.
• Maintain accurate records of patient care activities in compliance with ICD-10 coding standards.
• Documentation with EMR of PointClickCare.

Skills
The successful candidate should possess the following skills:

• Proficiency in appropriate practices and procedures.
• Strong understanding of Dementia care techniques.
• Knowledge of SNF settings
• Experience in LTC/ SNF medicine settings.
• Familiarity with care practices.
• Ability to perform Physical examinations effectively.
• Proficient in EMR documentation
• Competence in processing techniques.
• Familiarity with ICD-10 coding for accurate documentation.
