LPN (Licensed Practical Nurse)

Job Summary
We are seeking a compassionate and dedicated Nevada State Licensed Practical Nurse (LPN) with at least 3 years of home health experience to join our team in Las Vegas. The LPN will provide essential nursing care to patients, assisting with their daily needs and ensuring their comfort and safety. This role requires strong clinical skills, attention to detail, and the ability to work collaboratively with other healthcare professionals. The ideal candidate will be proficient for patient assessment, vital signs monitoring, and have experience in various care settings including senior care, home health.

Responsibilities

• Provide direct patient care, including administering medications and treatments as prescribed by physicians.
• Conduct patient assessments to monitor health status and report any changes to the nursing team.
• Maintain accurate medical records and documentation of patient care.
• Monitor vital signs and respond appropriately to any abnormalities.
• Collaborate with interdisciplinary teams to develop and implement patient care plans.
• Educate patients and families on health management, treatment plans, and wellness strategies.

Requirements

• At least 3 years of Home Health Care experience.
• Active LPN license in the State of Nevada (NV).
• Strong knowledge of medical records management systems.
• Ability to perform patient assessments effectively and efficiently.
• Proficient in monitoring vital signs and recognizing urgent health issues.
• Excellent communication skills, both verbal and written.
• Compassionate demeanor with a strong commitment to patient-centered care.
• Ability to work flexible hours.
