Position Highlights

Job Responsibilities

• Provide compassionate, individualized nursing care in the patient's home.
• Monitor the patient's condition and provide appropriate nursing interventions.
• Administer prescribed medications and treatments as directed by the patient's care plan.
• Maintain accurate and timely nursing documentation.
• Communicate with the patient, family members, and clinical team as needed.
• Ensure the patient's safety, comfort, and well-being.

Qualifications

• Current and valid Pennsylvania LPN license.
• Previous nursing or home care experience preferred.
• Dependable, compassionate, and professional.
• Strong communication and patient care skills.
• Reliable transportation to Canadensis, PA.
