Responsibilities

• Monitor and record vital signs of patients, ensuring accurate documentation.
• Assist in discharge planning by educating patients and their families on post-care instructions.
• Provide care for patients with infectious diseases, adhering to strict infection control protocols.
• Administer basic life support as required in emergency
• Deliver assisted living/memory care nursing care, including wound medication administration.
• Collaborate with healthcare team members to develop and implement individualized patient care plans.

Skills

• Proficient in monitoring vital signs and recognizing changes in patient conditions.
• Strong understanding of discharge planning processes and patient education techniques.
• Knowledgeable in infectious disease management and prevention strategies.
• Certification in basic life support (BLS) is essential.
• Ability to adhere to healthcare standards within various settings
