LPN (Licensed Practical Nurse)

1st Option Home Health, LLC is seeking a dependable and compassionate Licensed Practical Nurse (LPN) to join our Private Duty Nursing team and provide one-on-one care to a client in the home. We are looking for a Virginia-licensed nurse who works well with children, families, and other members of the care team. The ideal candidate is professional, patient, reliable, energetic, and committed to providing safe, high-quality care. Two years of nursing experience is preferred; however, qualified candidates with less experience are encouraged to apply.

Position Overview

The LPN will provide skilled nursing care in the client’s home under the supervision of a Registered Nurse and in accordance with the client’s physician orders and Plan of Care. Multiple shifts may be available, with opportunities for approximately 36 -40 hours per week. Weekend rotation may be required.

Responsibilities

• Provide one-on-one skilled nursing care in the client’s home
• Administer medications and treatments as ordered
• Monitor the client’s condition and promptly report changes in symptoms or behavior
• Perform and assist with G-tube care
• Perform and assist with tracheostomy care; training may be provided
• Safely use a Hoyer lift for transfers and mobility assistance ; training may be provided
• Assist with positioning, transfers, and activities of daily living as required by the Plan of Care
• Follow infection-control and patient-safety procedures
• Maintain accurate and timely nursing documentation
• Communicate effectively with the client’s family, RN supervisor, physicians, and other members of the care team
• Follow all agency policies, physician orders, and the established Plan of Care

Qualifications

• Current and unrestricted LPN license in the Commonwealth of Virginia
• Current CPR/BLS certification
• Current TB screening/PPD as required
• Experience with pediatric or private duty nursing preferred
• Experience with G-tube and tracheostomy care preferred
• Experience using a Hoyer lift preferred
• Ability to safely assist with transfers and mobility needs
• Strong clinical judgment, critical-thinking, and problem-solving skills
• Dependable transportation and ability to reliably commute to the client’s home
• Excellent attendance, punctuality, communication, and documentation skills
• Ability to build a positive and professional rapport with clients and families
